Children and Resident Encounter Program
The Medway Police Department has implemented the Children and Resident Encounter program (C.A.R.E.). This is a registry for individuals with Autism or other disabilities in an effort to give police access to critical information in the event of an emergency.
This information will provide first responders with emergency contact information, detailed physical descriptions, known routines, medications, favorite attractions, or special needs of an individual. This information can be critical for individuals with a mental or physical disability. The information provided can greatly assist first responders when time is essential in communicating and encountering an emergency involving a person with a mental or physical disability.
We ask that all questions be filled out completely and a current photograph be provided. If you are unable to supply a photograph, we can take one for you. The information you provide is confidential and will only be used by first responders. If any information on the registration changes you are encouraged to inform us as soon as possible.
Residents who could benefit from this program include but are not limited to,
- Individuals with Down Syndrome.
- Individuals with Alzheimer’s Disease.
- Individuals with limited mobility.
- Individuals who are non-verbal.
- Any Individual who is deemed at risk by their caregiver.
C.A.R.E Fillable Forms
This Children and Resident Encounter program is free to all residents. Please contact the Medway Police Department at (508) 533-3212 with any questions. Completed forms can either be e-mailed to Officer Benjamin Leazott , bleazott@medwaypolice.com , mailed to the Medway Police Department at 315 Village St. Medway, MA 02053 or handed delivered at the Medway Police Department Communications Center.
